BHLHE40 as a transcriptional rheostat: decoding microenvironmental signals in context-dependent tumor plasticity
The basic helix-loop-helix transcription factor BHLHE40 (also known as BHLHB2, DEC1, STRA13) functions as a critical molecular rheostat in cancer biology, exhibiting context-dependent roles that contribute to tumor plasticity. This review synthesizes current knowledge of BHLHE40, with emphasis on its structural features, including DNA binding, dimerization, and regulatory domains, as well as its integration of key physiological signals, such as circadian rhythm regulation, hypoxic adaptation, cellular senescence, and immune modulation. Pancancer analyses indicate that BHLHE40 expression is dysregulated across multiple tumor types. Elevated expression is associated with poor prognosis in cancers such as glioblastoma, acute myeloid leukemia, and ovarian cancer, whereas it correlates with favorable outcomes in breast and kidney cancers. Notably, the functional role of BHLHE40 is highly tissue-dependent. It promotes tumor progression in hepatocellular carcinoma, gastric cancer, colorectal cancer, and pancreatic ductal adenocarcinoma by enhancing proliferation, invasion, migration, epithelial-mesenchymal transition (EMT), and therapeutic resistance. In contrast, BHLHE40 exhibits tumor-suppressive effects in prostate cancer by inducing cellular senescence, and in endometrial cancer by inhibiting EMT. The tumor microenvironment plays a pivotal role in modulating BHLHE40 activity, with factors such as hypoxia, cytokine signaling, and immune cell interactions shaping its oncogenic or tumor-suppressive functions. Importantly, its role in maintaining CD8+ T-cell effector function during immune checkpoint blockade highlights its therapeutic potential. However, the context-dependent duality of BHLHE40 presents significant challenges for clinical targeting. Emerging therapeutic strategies, including targeted protein degradation approaches and advanced tumor models such as patient-derived organoids, offer promising avenues for intervention. Future studies integrating multi-omics data and spatial transcriptomics will be essential to fully elucidate the role of BHLHE40 and to harness its potential as a biomarker and therapeutic target for overcoming tumor progression, therapeutic resistance, and immune evasion.
