It is commonly associated with insulin resistance, hyperandrogenism, chronic low-grade inflammation, and oxidative stress. These factors contribute to metabolic dysfunction and increase the risk of developing type 2 diabetes and cardiovascular complications. This narrative review critically evaluates the available evidence regarding nutraceutical and herbal interventions targeting insulin resistance, inflammation, and oxidative stress associated with PCOS. A comprehensive literature search was conducted using PubMed, Google Scholar, EMBASE, Scopus, and CNKI databases, covering studies from database inception through 12 July to 21 November 2025 with keywords including PCOS, insulin resistance, hyperinsulinemia, oxidative stress, diabetes mellitus, nutritional supplements, and herbs. Relevant original research articles, randomized clinical trials, observational studies, systematic reviews, and meta-analyses were considered, and the available evidence was interpreted according to study design and evidence level. Evidence from preclinical studies and human clinical investigations indicates that selected nutraceutical and herbal interventions may influence metabolic, inflammatory, oxidative stress-related, hormonal, and reproductive parameters in PCOS. However, the magnitude and consistency of these effects vary substantially depending on the intervention, formulation, dose, treatment duration, participant characteristics, and the outcome assessed. Improvements in surrogate biochemical measures, such as HOMA-IR or inflammatory and oxidative stress biomarkers, should not be interpreted as direct evidence of improved fertility, reduced cardiovascular risk, or prevention of long-term diabetes. Therefore, the available evidence should be interpreted according to study design and evidence level, with greater weight given to findings from well-designed randomized clinical trials and systematic reviews or meta-analyses than to isolated preclinical or small clinical studies.","interpretation_basis":null,"fulltext_status":null,"interpretation":null,"fulltext":{"available":false,"status":null,"reason":"尚未抓取全文（未入选、分数未达阈值，或还没跑 fulltext 步骤）","pmcid":null,"license":null,"chars":0,"source":null,"fetched_at":null}}
